I am here to help coordinate appointments, assist with obtaining the right referrals and improve communication between clients and their providers. The goal is to make sure YOUR voice is heard. This can be done virtually or in person.
Preparing for Doctors’ Visits
Getting a serious diagnosis is often overwhelming! My services can include preparing concise questions to ask at your appointment so that proposed outcomes can be successfully achieved without missing important details. I will break down medical jargon into layman’s terms so that my client comes away from the visit with clarity and purpose. Bedside and other hospital visits can be done at the client’s request, according to pre-arranged fees.
Medication Management and Organization
My goal is to assist the client in understanding their medication regimen, drug interactions and keeping on schedule with prescribed regimens.
Organizing Medical Documents
​ I will obtain an in-depth history, consolidating all necessary documents into an easy to access and understand portfolio.
Resource Navigation
I will provide access to current resources pertaining to the client’s medical condition, allowing the client to explore alternative and approved treatment options.
Coordinating Healthcare Transitions
Services will include assisting clients and their families with managing healthcare transitions from hospital, to rehab, and finally to home.
Important Information
I am able to assist with explaining medical bills but will not be routinely providing assistance with reviewing claims, contacting insurance companies on my client’s behalf, or assisting with denials of service.
